How to Get a Medical Marijuana Card in WA
The first step in obtaining a MMJ card in Washington is scheduling a medical evaluation with a state-licensed doctor. You can get a copy of the Medical Cannabis Authorization Form online before your doctor’s visit. You must bring this form with you to a medically endorsed store! HWY 420 is a medically-endorsed store in the state. If you would like to receive a Medical Marijuana Recognition Card, you can also follow these steps:

2. Once you’ve received your Authorization Form from your Medical Provider, schedule an appointment to create your Recognition Card. (360)-627-8754
3. Once you’ve scheduled your appointment with us, all you need to bring with you is your Medical Marijuana Authorization Form and a Government Issued ID Card.
4. If you’d like to have a Designated Care Provider, please ensure that you bring this person with you to your appointment, and that they have their ID Card with them.
