If you would like to receive a Medical Marijuana Recognition Card, follow these steps:
1. Schedule an appointment at a medical practice that issues Medical Marijuana Authorization Forms. (If you’re not sure who to contact, give us a call and we’ll help you find a medical practice near you.) (360)-627-8754
2. Once you’ve received your Authorization Form from your Medical Provider, give us a call to schedule an appointment to create your Recognition Card. (360)-627-8754
3. Once you’ve scheduled your appointment with us, all you need to bring with you is your Medical Marijuana Authorization Form and a Government Issued ID Card.
4. If you’d like to have a Designated Care Provider, please ensure that you bring this person with you to your appointment, and that they have their ID Card with them.
